These forms are downloadable for your convenience. Please make sure you read all instructions

These forms are all in PDF and most are ‘fillable’, so you don’t need to print them. As long as you submit them from your @ufl.edu email address, you can type your name in the signature field or insert your digital signature file. If you don’t already have Acrobat Reader, you can download it from the Adobe site.

Application to Add or Cancel a Minor(PDF, 305 KB)
The minor affords a traditional, well-accepted way to recognize that a student has completed a significant body of work outside the major. Requirements for individual minors are available from the departments. You can find additional information on our Minors page.

Instructions: To add a CLAS minor, you should:

  • Complete the top portion of the minor form, and Sections 1 and 2.
  • Electronically sign the minor form.
  • If you are a CLAS major, email completed form from your @ufl.edu address to forms@advising.ufl.edu.
  • If you are not a CLAS major, email completed form from your @ufl.edu email address to your college.
  • Finalized forms will be forwarded to the Registrar’s Office for processing.

Combined Degree Program Request
Combined Bachelors and Masters Degree

Instructions: Email completed form with appropriate signatures (emails of approval from the departments’ @ufl.edu email addresses will be sufficient), forward all email approvals and the form to forms@advising.ufl.edu.

Innovation Academy Fall Petition Form (PDF, 466 KB)
Instructions: Please read the petition carefully. The petition has been updated and the instructions are in the form itself. Email completed form with appropriate signatures (emails of approval from the departments’ @ufl.edu email addresses will be sufficient: in lieu of a signature, your advisor can make a note in the Student Information System), forward all email approvals and the form to claspetitions@advising.ufl.edu.

University Dual Degree Application (PDF, 191 KB)
Dual undergraduate degrees in two different colleges, or two majors in CLAS that grant different degrees (e.g., a BS in Physics and a BA in Women’s Studies).
Instructions:

  • If Requested College is CLAS and both majors are in CLAS, you should:
    • Complete the top portion of the form, and Sections 1 and 2.
    • Email form from @ufl.edu email address to Undergraduate Coordinator (UGC) for the “Requested Major” (the one the student wants to add). UGC contact info can be found at the bottom of https://www.advising.ufl.edu/advising/who-is-my-advisor/.
      • The UGC will let you know if they cannot approve it, or if the UGC approves it, they will forward it on to the next approver in the process. You’ll be informed either way of the decision.
    • If Requested Major/College is CLAS, but your current major is NOT, you should:
      • Complete the top portion of the form, and Sections 1 and 2.
      • Email form from @ufl.edu email address to Undergraduate Coordinator (UGC) for the “Requested Major” (the one the student wants to add). UGC contact info can be found at the bottom of https://www.advising.ufl.edu/advising/who-is-my-advisor/.
        • The UGC will let you know if they cannot approve it, or if the UGC approves it, they will forward it on to the next approver in the process. You’ll be informed either way of the decision.
    • If Requested Major/College is not in CLAS and current major is in CLAS, you should:
      • Complete the top portion of the form, and Sections 1 and 2.
      • Email to the Requested Major/College (see contact list below).
        • The major/college will let you know if they cannot approve it, or if the UGC approves it, they will forward it on to the next approver in the process. You’ll be informed either way of decision.

    University Double Major Application
    Double majors (same degree, e.g., BA in History and a BA in Sustainability Studies) within the same college.

    Instructions: To apply for a double major in CLAS, you should:

    • Complete the top portion of the form, and Sections 1 and 2.
    • Email from your @ufl.edu email address to Undergraduate Coordinator (UGC) for the “Requested Major” (the one you want to add). UGC contact info can be found at the bottom of https://www.advising.ufl.edu/advising/who-is-my-advisor/.
      • The UGC will let you know if they cannot approve it, or if the UGC approves it, they will forward it on to the next approver in the process. You’ll be informed either way of the decision.

    Transient Request form – online at: http://www.floridashines.org.

    • Please allow 5-7 business days for your online form to be processed.
    • Before you complete this form, please contact the school to find out if you need to submit an application and/or additional materials first.
    • CLAS students taking courses out-of-state or at a private institution must fill out by hand the UF transient form and the Transient Equivalency form. Once you have filled out both forms and obtained department signatures (emails from the departments @ufl.edu email addresses will be sufficient), forward all email approvals and the form to forms@advising.ufl.edu.

    University Petition Information for CLAS Students
    The university petitions process is available for students to request a waiver of a university regulation or deadline. A university petition is used to request:

    • Retroactive Course Adds
    • Retroactive Course Drops
    • Retroactive Withdrawal (Nonmedical) from All Courses in a Term
    • Refund of Fees

    Petition to withdraw retroactively from a previous semester for medical reasons
    A Sub-Committee of the University Petitions Committee is available to consider petitions to withdraw retroactively from all classes from a previous semester for medical reasons only. If your retroactive withdrawal/drop is for selected classes or for non-medical reasons, please follow the guidelines above for University Petitions. Medical withdrawals include physical as well as psychological/emotional reasons, pertaining to the student or their immediate family, which includes mother, father, brother, sister. Immediate family may also include a spouse or child of the student.
